Background: In the U.S., nearly 500,000 infants are annually hospitalized in the neonatal intensive care unit (NICU) and over 90% survive to hospital discharge. After NICU discharge, these children are at high risk for medical and neurodevelopmental complications and require intensive medical and developmental follow-up. A successful transition is paramount to the health of the infants and the mental health of the caregivers.
