SUMMARY Paracoccidioidomycosis is a deep mycosis, that causes a sub acute or chronic granulomatous disease and involves skin, lymph nodes and internal organs. We report a 54 years old male coming from Paraguay that consulted for nausea, vomiting, asthenia and weight loss, lasting one year. An abdominal CAT scan showed a right adrenal tumor and a left adrenal hyperplasia. A percutaneous biopsy of the adrenal gland under CAT guidance disclosed a paracoccidioidomycosis. The patient was treated with amphotericin B, with good results.
