This article discusses the pivotal role of nursing in coordinating and designing practical and individual approaches to pulmonary rehabilitation following acute chronic obstructive pulmonary disease (COPD) exacerbation, as a means of preventing further functional decline, improving quality of life, reducing rates of hospital readmission, and lowering health care costs. Specific assessment guidelines at the time of acute exacerbation, recommendations for exercise and activity, and patient education to improve both knowledge and self-efficacy for participation in self-management of COPD, are the focus of the hospital to home rehabilitation program discussed here.
