Abstract 15938: In-hospital Outcomes of ST Elevation Myocardial Infarction in Centenarians

2017
Background: Population of centenarians (CN) has increased and ischemic heart disease (IHD) is found in >50% of CN at autopsy. There is no evidence-based guideline for management of ST-segment elevation myocardial infarction (STEMI) in CN. We evaluated trends in hospital management and outcomes of STEMI in CN. Methods: Nationwide Inpatient Sample database 2003-2011 contains 2427 CN (age 101±2 years, 81% women, 80% white, 57% hypertensive, 37% prior IHD, 12% diabetic, 8% hyperlipidemic, 2% smoker) hospitalized for STEMI. Results: Annual admissions decreased over study period (OR=0.89; 95% CI=0.876-0.91; p<0.001). Reperfusion therapy (RT) was performed in 3% [n=71 (percutaneous or surgical revascularization in 46 and 5, respectively; thrombolysis in 20)] and 97% were managed medically (MED). No significant peri-procedural complications were reported. RT were older (102±2 vs. 101±2 years), more often men (37% vs. 19%), white (100% vs. 79%) and more likely to suffer from chronic IHD and risk factors for IHD [h...
