Von Willberand Factor and Cellular Fibronectin Plasma Concentrations Are Increased in MRI-Positive Compared to MRI-Negative Traumatic Brain Injury Patients (P3.317)

BACKGROUND: Von Willebrand factor (vWf) is a marker of endothelial injury and cellular fibronectin (cFn) reflects integrity of blood brain barrier. We previously demonstrated that vWf and cFn are increased at the first hours after traumatic brain injury (TBI) compared to healthy controls. In this study we compare TBI patients with evidence of vascular injury on MRI with those without vascular injury and extend measurements through seven days of the injury. OBJECTIVE: To assess a relationship between vWf and cFn in patients with MRI-imaging positive and imaging negative TBI. DESIGN/METHODS: 55 TBI patients without trauma-related MRI findings (n=30) and with MRI findings (n=25) had blood collected within 24 hours and 7 days after the injury. Levels of vWf and cFn were measured in plasma by ELISA (Sino Biological, China and Biohit, Finland, correspondingly). RESULTS: Both on the 1st and the 7th day after the injury, vWf levels were increased in MRI-positive (42.9 ± 20.8 and 26.9 ± 41 mg/l, correspondingly) compared to MRI-negative TBI (28.1 ±15.4 and 24.5 ± 15.15 mg/l, correspondingly, (p=0.0159 and p=0.0275)). cFn was increased in MRI-positive TBI (4.03 ± 1.89 mg/l) compared to MRI-negative (2.75 ± 1.33 mg/l) at day 7 only (p=0.0296). There was a moderate correlation between day 1 vWf and day 7 vWf (Spearman r 0.496, p=0.0085), between day 1 vWf and day 1 cFn (r 0.676, p<0.0001) and between day 1 cFn and day 7 cFn (r 0.6852, p<0.0001). Receiver operator curve analysis demonstrated that vWf at day 7 moderately differentiated MRI-negative from MRI-positive TBI patients (Area Under Curve 0.7475, 95[percnt] CI 0.5570 to 0.9379, p=0.0276). CONCLUSIONS: vWf and cFn are increased in patients with MRI-positive findings compared to MRI-negative TBI subjects. vWf and cFn have different temporal profiles after TBI and have a potential as diagnostic and prognostic biomarkers after TBI.
