Editorial Comment: Controversy exists regarding whether varicocelectomy is beneficial for men with azoospermia due to spermatogenic dysfunction. These investigators compared men who underwent varicocelectomy 3 months before testis sperm extraction to those who underwent both procedures at the same time, and observed that those undergoing varicocelectomy before extraction demonstrated a higher retrieval rate. Although this study is subject to limitations such as a small size and the absence of a control group, it supports the practice of performing varicocelectomy before extraction if varicocelectomy is indicated and selected.
