2078 THE RELATIONSHIP BETWEEN NATURAL PASSAGE RATE OF LESS THAN 8MM URETER STONE AND C-REACTIVE PROTEIN AND NEUTROPHIL PERCENTAGE

INTRODUCTION AND OBJECTIVES: The ureter obstruction caused by ureter stone makes inflammatory change in proximal submucosal layer above ureter stone. It lead to interfere natural passage of ureter stone. Serum C-reactive protein (CRP) and neutrophil percentage are rising factor in the inflammatory response, is used as an indicator of inflammation in clinical practice. Thus, we analyzed the relationship between natural passage rate of less than 8mm ureter stone and C-reactive protein and neutrophil percentage. METHODS: This retrospective study was analyzed medical records of 187 patients with diagnosed ureter stone less than 8mm who underwent conservative therapy at our institution between January 2001 and January 2011. We defined ‘ureter stone removed’ means that the stone couldn’t find in radiologic exam after no specific treatment during eight weeks from diagnosed. Serum CRP divided three groups; 0 to 5.0, 5.1 to 9.9 and more than 10, neutrophil percentage divided two groups; normal group (38-74%) and abnormal group ( 74%), these factors were analyzed the relationship with ureter stone passage. RESULTS: In CRP subgroup, stone elimination rate was 94.1% (159/169) in 0 to 5.0 group, 70% (7/10) in 5.1 to 9.9 group, 50% (4/8) in more than 10 group. The higher CRP was associated with the lower elimination rate (p 0.001). In neutrophil subgroup, stone elimination rate was 94.5% (121/128) in normal group, 83.1% (49/59) in abnormal group. When neutrophil rate was normal, ureter stone elimination rate was significantly high (p 0.011). CONCLUSIONS: It helps to predict that measurement of serum CRP and neutrophil percentage whether the ureter stone passes spontaneously in less than 8mm sized ureter stone. Thus, when high CRP and neutrophil percentage, it is necessary to consider more aggressive treatment such as extracoporeal shockwave lithotripsy.
