Purpose Real-time tracking of clinical outcomes is limited by a lack of accuracy, consistency, and easy accessibility of longitudinal data. Increasingly available features of electronic health records (EHR), including standardized note templates and automated data extraction (ADE), offer improved clinical documentation and outcome tracking capabilities.
