Medical care according to risk level proposed by the new Spanish COPD Guidelines

Introduction: The new version of the Spanish Guidelines for COPD (GesEPOC 2017) proposes a new classification into two risk levels: low and high-risk. We aimed to characterize COPD patients according to this new classification and to evaluate if clinical assessments adapts to new recommendations. Methods: Prospective observational study. We included patients over one year (September 2017-2018). Demographic, clinical variables, and clinical phenotype were recorded. We analyzed adaptation of diagnostic and therapeutic intervention. Results: 430 patients: 66.5% high-risk; 33.5% low-risk. High-risk patients: worse lung function (%FEV1 44.61 versus 62.9); more previous exacerbations (1.81 versus 0.47); dyspnea mMRC≥2 (76.1% versus 36.9%), all p Conclusions: 2/3 of COPD patients were staged: high-risk and mostly of exacerbator phenotype. High-risk patients: more exacerbations, higher grade of dyspnea and worse lung function. Clinical activity in high-risk patients adapts to new recommendations. The most commonly prescribed treatment in high-risk patients is triple therapy.
